Enrollment Requirements

All uninsured members of a family, household, business or affinity group (for instance a club or church) must enroll. To exclude a family member from enrollment, please provide proof of that member's insurance.

Fees

Enrollment Fee

$65.00 per family unit.

Primary Care

Extended Laboratory & X-Ray Services - Additional Fee to Primary Care

Additional Fees

- A declined payment will incur a $50.00 'bad card' fee (enrollment may be immediately canceled).
- Reinstatement will require a new enrollment fee of $65.00.

Automatic Payments

DirectcareMD accepts payments ONLY by a pre-authorized, recurring charge to a single credit or debit card. This fee will be processed after each month the agreement is in force.
